3. Request for Continuance

A. Discussion and possible action on a request for continuance 
concerning Complaints 11-25 & 12-01, Board v. Nichols Funeral Home, Fairview , a formerly licensed Funeral Service Establishment, now terminated, and Jeremy Nichols a formerly licensed Funeral Director, now suspended, the former Funeral Director In Charge, for alleged Violations of 59O.S. § 396.12c. (13) and OAC 235:10-7-2 (14).

4. Matters requiring Administrative Hearing

A . Discussion and possible action on Administrative hearing concerning Complaints 11-25 & 12-01, Board v. Nichols Funeral Home, Fairview , a formerly licensed Funeral Service Establishment, now terminated, and Jeremy Nichols a formerly licensed Funeral Director, now suspended, the former Funeral Director In Charge, for alleged Violations of 59O.S. § 396.12c. (13) and OAC 235:10-7-2 (14). 

B. Vote on possible executive session pursuant to 25 O.S. Supp 1998, 
Subsection 307 (B) (8) to engage in deliberations or rendering a final Or intermediate decision in an individual proceeding pursuant to Article II of the Administrative Procedures Act regarding Nichols Funeral Home, Fairview, a formerly licensed Funeral Service Establishment, now terminated, and Jeremy Nichols a formerly licensed Funeral Director, now suspended, the former Funeral Director In Charge, for alleged Violations of 59O.S. § 396.12c. (13) and OAC 235:10-7-2 (14).

C. Possible action to be taken as a result of the executive session regarding Nichols Funeral Home, Fairview , a formerly licensed Funeral Service Establishment, now terminated, and Jeremy Nichols a formerly licensed Funeral Director, now suspended, the former Funeral Director In Charge, for alleged Violations of 59O.S. § 396.12c. (13) and OAC 235:10-7-2 (14).

5. Matters to consider possible consent orders 


A. Discussion and possible action on Administrative consent order concerning Complaints 11-25 & 12-01, Board v. Nichols Funeral Home, Fairview , a formerly licensed Funeral Service Establishment, now terminated, and Jeremy Nichols a formerly licensed Funeral Director, now suspended, the former Funeral Director In Charge, for alleged Violations of 59O.S. § 396.12c. (13) and OAC 235:10-7-2 (14).

6. Request for Administrative Hearing 

A . Discussion and possible action on a request for Administrative hearing concerning Emergency Order issued on August 14, 2012 Regarding the Crown Hill Funeral Home, Tulsa and Betty 
McConnell, Funeral Director in Charge for alleged Violations of 396.12 (B), 396.12c (17), 235:10-7-2 (17), and 235:10-15-3. 

B. Vote on possible executive session pursuant to 25 O.S. Supp 1998 Subsection 307 (B) (8) to engage in deliberations or rendering a final Or intermediate decision in an individual proceeding pursuant to Article II of the Administrative Procedures Act regarding Emergency Order issued on August 14, 2012, to Crown Hill Funeral Home, Tulsa and Betty McConnell, Funeral Director in Charge.

C. Possible action to be taken as a result of the executive session regarding Emergency Order issued on August 14, 2012, to the Crown Hill Funeral Home, Tulsa and Betty McConnell, Funeral 
Director in Charge.
